1 Isotretinoin is exact mechanism of action is unknown. Recent research suggests that the drug amplifies production of neutrophil-gelatinase associated lipocalin in the skin, which has been shown to reduce sebum production by inducing apoptosis in sebaceous gland cells while exhibiting an antimicrobial effect on Propionibacterium acnes.
The drug decreases the size and sebum output of the sebaceous glands. Isotretinoin combined impact on several of acne's contributory factors distinguishes isotretinoin from alternative remedies such as antibiotics and accounts for its greater efficacy in severe, nodulocystic cases. The effect on sebum production is temporary. However, remission of the disease can be "complete and prolonged. As a chemotherapy agent, isotretinoin may down-regulate the telomerase enzyme, inhibiting "cellular immortalization and tumorigenesis 2 Isotretinoin is a medication used for the treatment of a number of cancers and severe skin conditions. It was first developed to be used as a chemotherapy medication for the treatment of brain cancer, pancreatic cancer, and more. It is still used in the treatment of these cancers because of its ability to kill rapidly dividing cells.
Description:
1). Isotretinoin, also known as 13-cis retinoic acid is an oral pharmaceutical drug primarily used to treat cystic acne. Rarely, it is also used to prevent certain skin cancers (squamous-cell carcinoma), and in the treatment of other cancers. It is used to treat harlequin-type ichthyosis, a usually lethal skin disease, and lamellar ichthyosis. It is a retinoid, meaning it is related to vitamin A, and is found in small quantities naturally in the body. Its isomer, tretinoin, is also an acne drug.
2). Isotretinoin is primarily used as a treatment for severe acne. The most common adverse effects are a transient worsening of acne (lasting 2-3 weeks), dry lips (cheilitis), dry and fragile skin, and an increased susceptibility to sunburn. Uncommon and rare side effects include: muscle aches and pains (myalgias), and headaches. Isotretinoin is known to cause birth defects due to in utero exposure because of the molecule's close resemblance to retinoic acid, a natural vitamin A derivative which controls normal embryonic development.
3). Isotretinoin is used primarily for severe cystic acne and acne that has not responded to other treatments.Acne treatment usually begins with topical retinoids ,in combination with topical antibiotics or antiseptics , followed by oral antibiotics .In women a cyproterone acetate-containing contraceptive pill can be useful if there are no contraindications.
4). In cases of hormonal acne, such as in women in their 20s and 30s with cyclical acne, often a course of isotretinoin can permanently improve acne obviating the need for lifelong hormonal manipulation.
